.1Using the Collect email addresses setting is recommended for all forms patients complete. This setting allows the patient to have their responses sent to their email, which is a requirement for complying with E-SIGN Act.
.2The Send responders a copy of their response setting has three settings options in the dropdown: Off, When Requested, and Always.
.3Toggle off for patients to restrict editing a previously submitted form. For internal documentation, response editing can be turned on.
.4Toggle off to allow external use. We recommended turning this option on for internal documentation to restrict external users from completing the form. This also helps to ensure members of your organization are signed into their assigned accounts when completing forms.
.5Toggle off to restrict multiple submissions from the same patient.
