Google Forms

When creating internal and external forms within Google Forms, it is essential that the correct settings are configured for types of forms.
We recommend configuring your Google Forms settings, as illustrated in the screenshot above. Please see below for additional details for each form setting.
    .1Using the Collect email addresses setting is recommended for all forms patients complete. This setting allows the patient to have their responses sent to their email, which is a requirement for complying with E-SIGN Act.
    .2The Send responders a copy of their response setting has three settings options in the dropdown: Off, When Requested, and Always.
    .3Toggle off for patients to restrict editing a previously submitted form. For internal documentation, response editing can be turned on.
    .4Toggle off to allow external use. We recommended turning this option on for internal documentation to restrict external users from completing the form. This also helps to ensure members of your organization are signed into their assigned accounts when completing forms.
    .5Toggle off to restrict multiple submissions from the same patient.

Question Types

  • File Upload
  • This question type is unique as it will create a folder in Google Drive to store the uploaded file. If you are housing your Google Forms in a Shared Drive, this question type will not work. The File Upload question will only work if the Google Form is stored within an individual Google Drive.
  • Suppose you are storing Google Forms in a Shared Drive and would like this Form and folder to be accessible to your other staff. We recommend the following workaround:
  • Have an administrator store the form within their individual Google Drive accounts.
  • Create shortcuts for the Google Form and Upload Folder (this folder is linked to the Google Form) and place the shortcuts in your desired Share Drive.

  • Collect email addresses = Verified

  • Limit to 1 response = Toggle Off
